Death Certificate

  • Letter from applicant requesting for this. For death certificate requests, details of the deceased should include full name, date of birth, date of death, cause of death, place of burial and any other information that would assist the retrieval of the correct death certificate
  • Documents of identification to support the request – I.D, Driver’s licence, passport, photocopy of original birth, marriage or death certificate
  • US$35.00 money order/cashier’s check payable to the Embassy of Fiji
  • A reply-paid courier envelope (FEDEX/UPS, etc) to enable the Embassy to return your BC to you safely and promptly. Mail to the address below.
